Intracoronary thrombolytic treatment: another hazard.

چکیده
A patient with acute anterior myocardial infarction was treated with intracoronary thrombolysis. At cardiac catheterisation the first contrast injection into the left coronary artery dislodged some of the thrombus occluding the left anterior descending coronary artery. The thrombus embolised into the circumflex artery where fortunately it fragmented without causing occlusion. The risk of detachment of thrombus should be borne in mind when angiography and thrombolytic treatment are considered in acute myocardial infarction.

Cost benefit analysis of early thrombolytic treatment with intracoronary streptokinase. Twelve month follow up report of the randomised multicentre trial conducted by the Interuniversity Cardiology Institute of The Netherlands.
The costs and benefits of early thrombolytic treatment with intracoronary streptokinase in acute myocardial infarction were compared in a randomised trial. All hospital admissions were recorded and the functional class was assessed at visits to the outpatient clinic during a 12 month follow up of 269 patients allocated to thrombolytic treatment and of 264 allocated to conventional treatment. Me...
متن کاملThrombolytic, antithrombin, and antiplatelet treatment of acute coronary syndromes.
Acute coronary syndromes are a major public health problem and the leading cause of death in the western world. Acute coronary syndromes consist of unstable angina pectoris, non-ST-segment-elevation myocardial infarction, and ST-segment-elevation myocardial infarction. These diseases represent a continuum of increasing severity and are pathophysiologically linked to intracoronary thrombus forma...

Pump failure, ranging from ventricular dysfunction to acute cardiogenic shock, is now the leading cause of cardiac death. Efforts at temporary mechanical or pharmacologic support of the heart have been largely unsuccessful so that attention is now directed toward prevention of ventricular failure and limitation of myocardial infarct size or even outright prevention of infarction itself. In part...
متن کاملCombined thrombolytic and platelet glycoprotein IIb/IIIa inhibitor therapy for acute myocardial infarction: will pharmacological therapy ever equal primary angioplasty?
The management of acute myocardial infarction (AMI) was altered dramatically with the introduction of intracoronary thrombolytic therapy in the late l970s by Rentrop and others.1 The visualization of coronary artery occlusion by angiography performed during the first few hours of AMI and the removal of some of these thrombi at the time of emergent coronary artery bypass surgery convinced the me...
متن کاملOverview of Intracoronary Brachytherapy for in-Stent Restenosis of a Drug-Eluting Stent
Percutaneous coronary intervention with stenting is considered recently as the most common procedure for the treatment of symptomatic coronary. The article reviewed 41 studies published during 1997-2019 on intracoronary brachytherapy for in-stent restenosis of a drug-eluting stent. Intracoronary radiation therapy was finally confirmed in the setting of in-stent restenosis using as adjunctive th...
